We want to hear your views!
We have recently received funding from the Health and Social Care Alliance to help us work with local communities to find out what services and supports are available to people living with deteriorating health that enable them to “live right up to the end”.
Our goal is to work with local people to design services to support them to manage the physical, emotional and spiritual challenges they may face, and to help them to make plans for end of life.
In order to do this, we want to hear YOUR views.
We want to know:
- Have you ever thought that you might be in the last year of their life?
- If not, what might be important to you about that last year?
- What might you want to focus on, for you and for your loved ones?
- What gets in the way of you and your family thinking about and making plans for the future?
- What might help you?
- If you have thought about it what prompted you to think about it?
- What helped you to think and make plans?
- Do you have any suggestions of what we can do together in your community to help more people to live actively with deteriorating health and to make plans for the end of life?
We will be holding local meetings in late October and would like to know what day and time is most suitable for people, and if they require transport. We are also keen to speak to any community groups whose members may be willing to share their views.
